Home Care Partners, Inc. is a home health care and human services provider which has emerged as Minnesotans’ best option for better health. We provide home care services to a diverse population within the State of Minnesota.
Over the past decade, Home Care Partners, Inc. has earned itself the title of Minnesotans’ Option for Better Health, a partnership with deliverables. We are a locally owned and operated Comprehensive Home Care Agency providing services to Minnesotans of all backgrounds.
Our Goals
- To provide all levels of health and human services in collaboration with healthcare professionals to allow clients to remain in the comfort of their own homes as well as in the community while receiving care.
- To provide a broad base of services based on our clients’ needs, involving clients in establishing, implementing, and evaluating services.
- To allow clients to remain in their homes as long as possible by providing care and related health and human services.
- To assist clients in using all restorative methods and procedures to return to their regular pattern of living as soon as it is possible.
- To provide cost-effective, cost-competitive, and high-quality health and human services to all clients.
- To develop a relationship with referral sources who serve as our partners to effectively meet the healthcare needs of all clients.
- To serve as a link between clients and available community resources in the least restrictive way to meet their needs.
Our Care Ethics
We combine faith with compassion to ensure we maintain the golden rule of treating others with dignity and respect while treating others the same way we would want to be treated if we were in their particular situation. “Do onto others as you would have them do onto you” is a constant reminder for all caregivers as they carry out their responsibilities.
Our Experience
Our care team is composed of highly trained and efficient personnel who are highly dedicated to providing our clients and their families with the best comprehensive and person-centered services.
Every client is assessed, and a care plan is developed to meet the distinct services needed. With the understanding of the complexities of home care, we strive to balance each person’s needs with safety to minimize or eliminate risks in our care delivery experience. For this reason, Home Care Partners, Inc. recruits staff members with adequate experience and skills to meet clients’ needs.
Our Staff/Care Team
Our Care Team is made up of diverse individuals who have the expertise and training to have Home Care Partners, Inc. meets its objectives, therefore meeting clients’ expectations. Our Care Team works in a collaborative fashion to ensure that all the care needs of all clients are adequately met. We have both licensed and non-licensed staff providing adequate home care services to clients based on each client’s specific plan of care. Our Administrative Team provides oversight and on a professional basis has adopted the Servant Leadership style by which Home Care Partners, Inc. is adequately managed to remain in compliance with laws and statutes governing the health care industry. All teams, as well as our clients are encouraged to ascribe to the TEAM acronym wherein working “Together Everyone Achieves More.” Our Care Team embodied the following:
- Registered Nurses (RNs)
- Public Health Nurses (PHNs)
- Licensed Practical Nurses (LPNs)
- Certified Nursing Assistants (CNAs)
- Home Health Aides (HHAs) *
- Personal Care Assistants (PCAs) *
- Direct Support Professional (DSP) *
- Homemakers *
*All non-licensed staffs are directly supervised by registered nurses to ensure compliance in meeting clients’ needs.
Our Administrative Team provides complete oversight and on a professional basis has adopted the Servant Leadership style as expressed by Robert Greenleaf with which Home Care Partners, Inc. is adequately managed. The Servant Leadership at all levels of the organization focuses on service first. We are servants first, primarily focusing on the growth and well-being of the clients we serve, and everyone else within the organization as well as each of the community to which we belong. Teams at all levels are encouraged to lead by example, and to remain in compliance with laws and statutes governing the home care industry. All teams, as well as our clients are encouraged to ascribe to the TEAM acronym wherein working “Together Everyone Achieves More.”
All our staff and employees are properly screened to ensure continuity at all levels and at all times. Home Care Partners, Inc. is a partner to the State of Minnesota vigorous screening process which all fully veted employees MUST successfully complete and pass the required background checks. In addition to this process Home Care Partners, Inc. also takes pride in the level of its management expertise in properly vetting potential employees that meet the needs of our the clients we serve and also buy into its mission and vision.
